FINANCIAL POLICY
We require payment in full for any amounts designated to be the patient’s responsibility at the time services are rendered. Any outstanding balances will be billed to the patient. If an overpayment occurs, a refund will be processed once all outstanding balances are settled.
NO SHOW POLICY
At Botox in the Park, our goal is to provide quality and safe medical care. The following policy is with regard to patients who fail to keep their scheduled office visit appointment or procedure appointment. Please be courteous and call us promptly if you are unable to attend an appointment. Patients who fail to show for their scheduled appointment or did not notify the office within 24 hours of their scheduled appointment time shall be subject to a “No Show/Cancellation” fee of $150. In the event of an actual emergency (with documentation provided) and prior notice could not be given, consideration will be given. To cancel or reschedule appointments feel free to contact us via phone or text message at Botox in the Park at 206-717-5622, 24 hours a day. If you have any problems getting through, you can leave a message with your name, appointment date, and cancellation reason or request for rescheduling.
INSURANCE & BENEFITS
We work with patients to help navigate their insurance benefits. Please contact our office with your insurance information, and our team will be happy to investigate your benefits and help you understand your coverage.
Please note that insurance coverage and benefits vary by plan, and verification of benefits does not guarantee payment.
PATIENT AS A PARTY OF LITIGATION
If you are party to a litigation or auto accident case, we require an attorney’s letter of representation.
PAYMENT ARRANGEMENTS
All payment arrangements made prior to the date of service are calculated based on the closest possible estimate of patient responsibility. A prearranged payment arrangement may need to be modified if the final patient balance differs from the original estimate. All patients with payment arrangements that are defaulted will receive three internal collection letters prior to receiving outside collection activity. If a previously defaulted patient resumes payments and again defaults, they will receive one notice prior to receiving outside collection activity.
PAYMENT OF POST-VISIT PATIENT BALANCES
All past-due balances and balances above original point-of-service estimates must be paid within 30 days of when the balance becomes the patient’s responsibility. An acceptable payment arrangement may be made in order to prevent outside collection activity. If your account becomes past due, we will take the necessary steps to collect this debt. A $25.00 monthly statement fee will apply for any balances not paid within 30 days of the initial statement. If we have to refer your account to a collection agency, a $100 collection agency fee will be added to your outstanding balance.
